Changes in concentration and mental clarity can have several explanations, including poor sleep, emotional strain, medications, and some hormonal conditions. Understanding the pattern helps guide an assessment.

“Brain fog” describes a feeling. The details help explain it.
People use “brain fog” to describe experiences such as difficulty concentrating, feeling mentally slower, or struggling to recall information. It is a description rather than a diagnosis.
Try to identify what has changed from your usual pattern and how it affects your day.
- Is it harder to stay with a task or conversation?
- Are you forgetting details you would normally remember?
- Does your thinking feel slower at particular times?
- Did the change begin alongside an illness, new medication, or disrupted sleep?
The hormonal connection
Hormonal conditions can affect concentration.
Thyroid function and mental slowing
An underactive thyroid can cause difficulty concentrating or thinking clearly. Fatigue, feeling unusually cold, constipation, or weight gain may occur alongside it.
Your physician considers the combination of symptoms and may use thyroid blood tests to investigate.
Testosterone and concentration changes
Difficulty concentrating can occur with testosterone deficiency, sometimes alongside reduced libido, low energy, or changes in mood.
These symptoms are not specific enough to diagnose low testosterone. A physician considers your history and appropriately measured blood levels.
What testosterone research shows
A randomized trial studied men aged 65 and older with low testosterone. Among 493 participants with age-associated memory impairment, one year of testosterone treatment did not improve memory or other measured cognitive abilities compared with placebo.
This study does not answer every question about younger men or different causes of brain fog. It does mean we should not promise that TRT improves memory or focus.
Assessment helps determine whether a hormonal condition needs treatment and what else deserves investigation.

Hormone blood tests can identify some contributing conditions. They do not provide a complete assessment of memory or attention, and further evaluation may be needed.

Can low testosterone cause brain fog?
Difficulty concentrating can occur with testosterone deficiency, but “brain fog” has many possible explanations. Your physician considers other symptoms and blood results before deciding whether low testosterone is involved.
Will TRT improve my memory or concentration?
That cannot be promised. A major randomized trial in older men with low testosterone and memory impairment did not find cognitive improvement with treatment.

Can thyroid problems affect how clearly I think?
Yes. Difficulty concentrating or thinking clearly is a recognized symptom of an underactive thyroid. Other symptoms and thyroid blood tests help determine whether this is relevant to you.
Is brain fog the same as ADHD?
No. “Brain fog” describes a subjective experience; ADHD is a distinct clinical diagnosis. Concentration difficulties can overlap, but a hormone test cannot diagnose ADHD. Mention whether the difficulties are longstanding or represent a recent change.
What if my hormone results are normal?
Your concern still deserves attention. Your physician may consider sleep, emotional wellbeing, medications, or other causes and discuss whether further assessment is appropriate.

Yes. Complete the health assessment with the information you have and share any existing results. Our team explains the process, and your physician determines which investigations are relevant.
